Exclusive (EPO)
The choice for employees wanting the most benefits at the lowest costs. These benefits include low co-pays for physician services, inpatient and outpatient services, for routine physical exams, and for your prescription drug benefit as well. The EPO option requires 100 percent utilization of Cox Health Systems and Primrose physicians.
Preferred (PPO)
The option for employees who may want a higher level of choice at a modestly higher cost. This option allows for increased levels of benefits for utilizing in-network providers, but also provides benefits when using out-of-network providers.
Indemnity
The option for those employees wanting the highest level of choice--in a traditional 80/20 co-insurance benefit plan.
