Cox Comprehensive Inpatient Rehabilitation

*Community is patient discharged home or residential care.
*LTCF is patient discharged to Long Term Care Facility.
*Acute is patient discharged to hospital setting.
*Rehab is patient discharged to extended rehab setting.
Average number of hours of therapy per day is 2½ hours, 7 days per
week. Most patients were admitted to the rehabilitation unit requiring at
least minimal assistance with dressing, bathing, toileting, transfers and
walking. Most patients were discharged able to perform these activities
independently with the assistance of equipment or taking extra time.
Satisfaction survey results indicate that overall patients were
satisfied with the care provided during the program. A ranking of 4.38 out
of a possible 5 on the Likert scale was received.
Notice to all patients with Medicare as the primary payor. Medicare is
requiring information be recorded and sent for all rehab stays being paid
for by Medicare. The patient assessment information being reported is done
to provide Medicare with information regarding how much help you need with
self cares, mobility and cognitive skills (thinking and memory tasks) at
the time of admission compared to how well you are doing at the time of
discharge. The other information being collected and sent is who you are
(identification information) and the medical reason for being transferred
to rehab as well as other medical conditions you have as recorded by your
physician. The information collected will be transmitted electronically to
Medicare by a confidential and secure line. These rights include: · The
right to be informed of the purpose of the patient assessment data
collection; · The right to have any patient assessment information that
is collected remain confidential and secure; · The right to be informed
that the patient assessment information will not be disclosed to others
except for legitimate purposes allowed by Federal Privacy Act and Federal
and State regulations; · The right to refuse to answer patient assessment
date questions; and · The right to see, review and request changes on the
patient assessment instrument.
**Regional and National data are compiled using reports
from the Uniform Data System for Medical Rehabilitation covering
01-01-03
through 12-31-03.
