Cox Rehabilitation Services
Planning for the patient’s return to home, work, school
and community begins the day the patient enters the Cox Rehabilitation
unit. Tentative discharge dates are established at the time of admission.
To help patients transfer the skills they learn in the rehabilitation
process to less structured surroundings, actual and simulated situations
are used in therapy. Patients are given the opportunity to apply problem-solving
techniques and demonstrate functional skills of daily living in various
settings.
We identify problem areas and skills that require additional
development to ease the transition from our rehab program to the patient’s
home. Home visits are included to identify safety and accessibility issues.
In addition to suggested
safety tips, the home, school or work environment may require
modifications or specialized equipment in preparation for the patient’s
return.
Follow-up Care
Once our patients have completed their rehabilitation stay,
they are evaluated for the need of follow-up services. A coordinated plan
of care for life after discharge is implemented and services are identified
to help patients maintain and improve their health. Rehab therapy may
be continued on an outpatient
basis, home health care
may be arranged or patients may be referred to other community resources.
